At Journey Family Medicine, we are focused on providing inclusive care that addresses your primary health, behavioral health and your social needs. Whether you’re new to our community or seeking a new provider, we’re here to welcome you and take care of you like family. Follow these step-by-step instructions for becoming a new patient.
If you are interested in becoming a patient at Journey Family Medicine, please complete our new patient pre-registration form. Once we receive your completed new patient pre-registration form, we will process it and contact you as soon as possible.
Once you are accepted as a patient, and before your first appointment, please complete our online patient forms. Alternatively, you can download, print and complete the forms by hand, then bring them to your first appointment. Get started.
For your convenience, we offer both telemedicine and in-person appointments, depending on your preference and the reason for the visit.

At your first appointment, we’ll spend some time talking about your medical history and family health history, as well as any behavioral or mental health issues. It’s important for us to get to know you and have a clear picture of your life and health goals to develop a plan of care.
We accept most insurances. You are encouraged to contact your insurance provider directly if you have questions about your insurance eligibility, prior authorization, coverage, deductibles, premiums and copays. Patients or guardians of patients are ultimately responsible for all payment obligations related to treatment or care and are asked to guarantee payment for our services. Copays are due the day of your appointment.
